PAY RANGE: Band H NP/PA ($75,000-$91,250/year), Band J MD ($125,000-$143,750/year)

YOUR ROLE & IMPACT

The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. CRCH seeks an experienced primary care provider for a dynamic dual role: Serve as the go-to person for time-sensitive clinical needs during clinic hours, while mentoring early-career primary care providers. The aim is to build their professional confidence, support integration, and foster development. The Provider of the Day will be the designated medical provider for support staff to consult on clinical decisions for patients whose primary providers are unavailable, particularly in urgent situations.. The Provider of the Day will also meet with medical providers to share their experience and provide guidance to help build the providers’ clinical and non-clinical skills and confidence. As a Federally Qualified Health Center, CRCH’s patients are primarily from underserved communities, insured by affordable healthcare options, with English as a second language. Given this, an important component of the Provider of the Day role is mentoring providers to develop strategies to successfully deliver high-quality care to traditionally disenfranchised communities, while also supporting the health center's financial and quality goals.

TIME EXPECTATIONS:

  • 20-25 hours/week on-site at Brighton, MA site
  • Of the above listed hours, 16-20 clinical hours and 4-5 admin hours per week

YOUR RESPONSIBILITIES

As the Provider of the Day, you will not have a patient panel, allowing you to be available for time-sensitive requests and emergency situations. On the days you work on-site, your responsibilities will include:

  • Respond to clinical questions from nursing and support staff, ensuring continuity of patient care when the assigned PCP is unavailable (e.g., re-ordering labs, addressing paperwork requests).
  • Addresses urgent clinical needs for patients of other PCPs not working that day (e.g., addressing critical lab results, re-ordering specialty referrals close to expiring).
  • Offer guidance to nurses triaging same-day patient needs, providing input on clinical issues that require experienced provider assessment (e.g., determining if patient symptoms warrant an ED visit).Lead the clinic's emergency response by assessing the situation, providing immediate intervention, and coordinating with nursing and support staff to stabilize the patient.
  • Inspire early-career providers to expand their clinical knowledge and encourage curiosity-driven learning and decision-making.
  • Provide targeted interventions and feedback to mentees to enhance their growth and confidence.
  • Ensure timely documentation and follow-up care for emergency cases, collaborating with other healthcare providers as necessary.
  • Serve as the designated provider for the rapid response team, addressing emergency clinical issues as they arise (e.g., a patient experiencing a medical emergency on-site such as fainting, chest pain, or sudden deterioration).
  • Maintains a flexible appointment schedule to accommodate same-day bookings for staff when other time slots are unavailable.
  • Conduct weekly one-hour meetings with a select group of new or early-career MDs, NPs, and PAs to discuss career development, review patient case scenarios, and provide guidance on managing challenging patient interactions, such as prioritizing concerns during same-day urgent care visits.
  • Hold monthly check-ins with the Chief Clinical Officer (CCO) to discuss mentorship progress and clinical initiatives.
  • Provides individual didactic education to mentees upon request and provides guidance on resources available to build clinical confidence.
  • Perform additional duties assigned by the CCO, or designee.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Primary care MD with 3-5 or more years of experience, or primary care NP or PA with 5-7 or more years of experience who would like to positively contribute to the development of early career primary care providers entering the workforce.
  • Strong ability to coach and mentor providers; use discernment when intervening to provide coaching to PCPs.
  • Must hold a valid, clean, and active Massachusetts MD, NP or PA license, maintain a current Massachusetts Controlled Substance Registration (MCSR) and DEA registration, and be eligible for Full Practice Authority in Massachusetts.
  • Must complete continuing education to maintain licensure.
  • Must apply for and be granted privileges at affiliated hospital(s) (MDs only).
  • Understanding of ambulatory family practice health care program administration and clinical operations preferred.
  • Experience responding to medical emergencies in a clinical setting, with the ability to make quick, decisive interventions to stabilize patients in urgent situations.
  • Experience with the Patient-Centered Medical Home model and clinical operations in an ambulatory setting preferred.
  • Familiarity with Epic EHR and Microsoft Office systems preferred.
  • Must be able to work on-site at our Brighton location.
  • Fluency in Spanish and/or Portuguese a plus; bilingual/bicultural preferred.
  • Strong communication skills, with the ability to lead with emotional intelligence and compassion.
