Pre-Licensure Staff Nurse

CLASSIFICATION/STATUS: Non-Exempt, Full Time, Licensed within 1 year of hire

IMMEDIATE SUPERVISOR: Nurse Manager

SUPERVISORY RESPONSIBILITIES: None

PAY RANGE: Band B ($20.50 - $26.00/hour)

The Pre-Licensure Staff Nurse provides direct professional nursing care and education to health center patients and ensures continuity and quality of care under direct supervision. The Pre-Licensure Staff Nurse has a solid understanding of patient needs and will work collaboratively with primary care providers and a multidisciplinary team to develop, assess, coordinate, and optimize patient care plans under direct supervision.

Direct supervision includes, but is not limited to, the supervising licensed nurse being physically present in the health care practice setting and readily available where the Pre-Licensure Staff Nurse is practicing.

YOUR RESPONSIBILITIES

  • Function effectively and safely under the direction and supervision of a licensed nurse, performing tasks within the scope of the supervising nurse of equal or higher education.
  • Pre-Licensure Staff Nurse must seek assistance from the supervising nurse immediately when they encounter patient care situations that are beyond their competency and level of academic preparation.
  • Perform a comprehensive and focused health assessment that includes health history and complete physical evaluation; considers the patients’ psychosocial, emotional, ethnic, cultural and spiritual dimensions of health, including his or her understanding of their health/illness experience and how their daily life is affected.
  • Use and adapt assessment tools and techniques based on the patients unique needs.
  • Complete and document a thorough medication assessment.
  • Document health assessment and clinical data in visit note and patient profile or summary as appropriate. Provide culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.
  • Provide health education to facilitate patient treatment plans.
  • Actively empower patients and families to effectively manage their health care and navigate the health care system.
  • Communicates with members of the health care team to ensure continuity in care transitions.
  • Review and follow-up with patients and members of the health care team to ensure that patients are current with needed care.
  • Screen all calls and assist with walk-ins. Prioritize care based on acuity when organizing patient needs.
  • Maintain awareness of clinic flow and re-arrange provider schedules to accommodate urgent care needs.
  • Demonstrate knowledge of community resources and assist staff, patients, and families to effectively utilize them.
  • Ensure effective coordination of care across the care continuum, including home care, outpatient clinics, emergency/urgent care, and hospital settings.
  • Facilitate the advanced care planning process, assisting patients and families to consider options and goals for future healthcare decisions. Document advanced care goals in the EHR.
  • Serve as a resource person to other members of the team around the care needs of patients and provide support to other members of the clinical and administrative team in an accessible, open manner.
  • Delegate safely and effectively to support staff tasks that are within scope of practice.
  • Communicate clearly and effectively with the care team about patient status and needs.
  • Collaborate with other Health Center staff to meet JCAHO and other regulatory and/or funding requirements.
  • Perform other duties as required by Nurse Manager or designee.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Must have graduated from a board approved nursing education program within one year of the hire date or be a student currently enrolled in their last semester of a board approved nursing education program.
  • Must obtain Massachusetts LPN or RN license within 1 year of hire.
  • Excellent communication and documentation skills required.
  • Certification in CPR required.
  • Must have knowledge base including basic health teaching, family planning education, and how to care for common pediatric and adult illnesses.
  • Must be willing to educate and mentor clinical support staff as needed.
  • Must be willing to work one evening per week and one Saturday per month.
  • Must have basic computer skills preferred or willingness to develop these skills.
  • Experience with telephone assessment and crisis management preferred.
  • Bilingual (English/Spanish or English/Portuguese) preferred.
  • Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
  • Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
  • Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.

***The nurse is not permitted to practice under this authorization if they receive a failing score on the NCLEX examination.
